Position Summary

Ensure every patient receives a thorough, comprehensive, and individualized physical therapy regimen to ensure optimal patient outcomes.

Compensation: $70,000 - $80,000

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

· Perform a thorough evaluation of incoming patients to curate and document a recommended treatment plan based on each patient’s individual needs, consulting with program director and/or referring providers as needed.

· Provide exceptional, ongoing care to patients, assisting with all phases of physical therapy treatments and techniques.

· Discuss treatments with patients, recording their conditions and responses to therapy in their medical record after each visit.

· Complete daily patient care notes, progress notes, discharge notes, and other required forms as requested by referring providers, rehabilitation nurses, or insurance companies.

· Collaborate with both scheduling and authorizations teams to help ensure fluid treatment progression.

· Operate and apply knowledge of standard physical therapy equipment.

· Maintain friendly, cordial relations with all patients, vendors, and colleagues.

· Comply with state and Company standards of operations and

· Adhere to the Company’s core values and H.O.P.E. mission statement.

· Regular participation and measurable involvement in marketing and/or business development efforts.

· Additional duties as assigned.

Required Qualifications

· Master and/or Doctorate degree in Physical Therapy from an accredited program.

· Appropriate PT licensure and Colorado state registration (in good standing).

· Interest in pain management, including chronic, highly complex patients.

Minimum Qualifications (Knowledge, Skills, and Abilities)

· 2-3 years with high medical complexity patient populations.

· Certification in dry needling is preferred, but not required.

· Knowledge of interventional pain management procedures is preferred, but not required.

· Excellent oral, written, and listening communication skills.

· Effective organization and time management skills with the ability to work in high-pressure situations.

· Excellent interpersonal skills and demeanor.

· Proficient in the use of a PC; in a Windows environment; in the use of the Internet; and in the use of MS Office Applications such as Outlook, Word, PowerPoint, and Excel.

· Proficient in the use of EMR systems such as Modernizing Medicine (“EMA”).

  • Ability to work both independently, as well as collaboratively.
  • Ability to identify and proactively solve problems.
  • Ability to thrive in a patient-focused environment.
